PRESCRIPTIONS & REFILLS

Patients are responsible for keeping track of remaining refills and scheduling appointments accordingly. A patient must be seen within three months to reauthorize prescriptions. Refills will typically be handled during your appointment. Prescription refills are not available after 5pm during the week, on weekends, or holidays.

If you are in need of a prescription refill that is outside of your scheduled appointment, complete the refill request form below. Please allow 72 hours for prescriptions refill requests to be processed. A $100.00 will be charged for refills called in between appointments.
